So I had this doctor appointment scheduled for me 3 weeks ago, now I come here and the waiting room is full, nowhere to sit. Why they do this

A doctor friend once responded to this kind of complaint: if you’re complaining about this, then you’re probably in the lucky part of the population for whom doctor visit is just routine. @Gargron you’ll have to explain the term "ER". What I said is true at least in France (where I live)
@immae Emergency Room in a hospital, where you go when there's no time to get an appointment
@Gargron @immae Well, family doctor clinics do take walk-ins as well -- people who need (sometimes prompt) attention, but do not have emergencies. This is good because ot keeps hospital emergency rooms free for real or severe emergencies.
I believe Mondays are especially busy at clinics because people who got sick or had an issue arise over the weekend come in the next business day.

@immae This isn't ER and they are the ones planning the appointments. It's an organization issue.
